Job summary

Care Pavilion Nursing & Rehab Center is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join our full-time therapy team in a SNF/LTC setting. You will provide targeted OT treatments, help residents regain independence in daily living activities, and contribute to individualized care plans.

The role emphasizes collaboration with a multidisciplinary team, ongoing documentation in Net Health, and a commitment to resident-centered outcomes within a supportive leadership

Qualifications

• Licensed COTA in good standing with the state license or eligibility for licensure.

Responsibilities

• Provide occupational therapy treatments under the direction of the OT.
• Assist residents with daily living skills to improve independence.
• Help implement individualized therapy plans for each resident.
• Document therapy sessions and progress using Net Health.
• Collaborate with the interdisciplinary care team to meet resident goals.
• Encourage residents through creative and functional therapy interventions.

What You’ll Do

• Provide occupational therapy treatments under the direction of the Occupational Therapist

• Help residents improve independence with daily living skills such as dressing, grooming, mobility, and self-care

• Assist in implementing individualized therapy treatment plans

• Document therapy sessions and resident progress using Net Health

• Work closely with the interdisciplinary care team to support resident goals and outcomes

• Encourage residents through creative and functional therapy interventions

What We’re Looking For

• Licensed Certified Occupational Therapy Assistant (COTA)

• Active state license (or eligibility)

• Experience in SNF/LTC preferred, but motivated candidates are encouraged to apply

• Strong communication and teamwork skills

• A positive attitude and passion for helping others regain independence
