Job summary

Care Pavilion Nursing & Rehab Center is seeking a Certified Occupational Therapy Assistant (COTA) to join our full-time therapy team in a SNF/LTC setting. You will provide therapy under the Occupational Therapist, assist residents with daily living activities, and help implement personalized treatment plans.

Strong communication and teamwork are essential as you document progress in Net Health and collaborate with the interdisciplinary care team to achieve resident goals and meaningful outcomes.

Qualifications

• Licensed Certified Occupational Therapy Assistant with an active state license or eligibility.
• Experience in SNF/LTC setting is preferred but motivated candidates are encouraged to apply.
• Strong communication and teamwork skills and a patient-centered approach.

Responsibilities

• Provide occupational therapy treatments under the direction of the Occupational Therapist.
• Help residents improve independence with daily living skills such as dressing, grooming, mobility, and self-care.
• Assist in implementing individualized therapy treatment plans.
• Document therapy sessions and resident progress using Net Health.
• Work closely with the interdisciplinary care team to support resident goals and outcomes.
• Encourage residents through creative and functional therapy interventions.

What You’ll Do
