Job summary

Cornerstone Rehab is hiring a full-time Certified Occupational Therapy Assistant to deliver care to our patients and work with a diverse team of clinical professionals. This role is based in Kentucky and may require travel to nearby centers.

Qualifications

• Valid COTA License and Credentials.
• SNF and/or Acute care experience.

Responsibilities

• Provide occupational therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care in accordance with the patients' needs and capabilities.
• Travel to nearby centers as needed.
