Job summary

Cornerstone Rehab is hiring a full-time Certified Occupational Therapist Assistant to deliver care to adult and geriatric patients and collaborate with a diverse team of clinical professionals in our Kentucky facilities.

You will travel to nearby centers as needed, prepare documentation and discharge summaries, and establish a care plan aligned with patient needs. Cornerstone Rehab offers a supportive environment and growth opportunities for licensed therapists.

Qualifications

• Valid COTA license and credentials required.
• SNF and/or acute care experience preferred.

Responsibilities

• Provide occupational therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care based on patient needs and capabilities.
• Travel to nearby centers as needed.
