Job summary
The Legacy Senior Communities seeks a Certified Occupational Therapy Assistant to support assessment, planning, implementation, and documentation of OT programs under supervision. - Must have current Cardio Pulmonary Recitation Card (CPR) certificate from a recognized provider or obtain CPR within 30 days of hire.
Responsibilities
- Assess and screen residents for OT intervention and communicate results to families and staff.
- Plan and implement therapeutic activities to meet patient goals and coordinate with the care team.
- Document daily interventions and discharge planning as needed.
- Collaborate with the care team to ensure appropriate and timely discharge or transfer.

The Certified Occupational Therapy Assistant Assist with assessing, planning, implementing and documenting member’s, resident’s or patient’s occupational therapy program under the direction and supervision of the Occupational Therapist. Follow community guidelines, objectives and policies to maintain standards of treatment consistent with legal and accrediting requirements.
The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.
- ASSESSMENT: Upon receiving OT screening request, review member, resident or patient’s current situation and screens individual for intervention. Communicate screening results to member, resident or patient and appropriate family and/or staff. Complete required screening documentation.
- PLANNING: Plan appropriate therapeutic activities to meet client’s goals. Communicate treatment plan and goals to client and family. Communicate significant issues in treatment planning to OT on ongoing basis. Collaborate with health care team members to begin planning for appropriate and timely discharge or transfer.
- IMPLEMENTATION: Demonstrate ability to prioritize treatment procedures according to patient’s needs and MD requests. Implement interventions according to plan of care. Communicate purpose of treatment procedures to member, resident or patient and/or family according to level of understanding and modifies plan accordingly. Communicate ongoing treatment issues/plan to interdisciplinary health care team members. Communicate ongoing treatment issues with OT. Complete required documentation regarding daily intervention. Demonstrate awareness of precautions regarding client diagnosis and treatment techniques.
- EVALUATION: Review and evaluate effectiveness of interventions toward achieving patient outcomes and discuss need for changes to OT. Document appropriate reassessment and changes in plan of care. Communicate significant changes in client status and therapeutic intervention to member, resident or patient and/or family and health care team members. Communicate with OT when discharge from therapy appears appropriate. Complete required documentation for discharge.
- OTHER DUTIES: Perform other related duties as assigned. Support and participate in changes and suggestions made by Director of Therapy
- GENERAL FUNCTIONS: Maintain confidentiality of resident, employee and facility information. Follow Resident’s Rights policies at all times maintaining privacy, confidentiality and respect. Ability to understand and comply with applicable Federal, State and local regulations as they apply to this job. Attend all required department and/or facility meetings. Adhere to applicable laws, rules, regulations and Code of Conduct directed by Corporate Compliance Program, conduct business using ethical standards, and report any suspect or actual event or circumstance to the Compliance Officer. Understand and comply with use and disclosure rules of resident Protected Health Information (PHI) as required by the HIPAA Privacy Rule of the Health Insurance Portability and Accountability Act. Use the proper equipment for each job and report any missing or damaged equipment to supervisor immediately. Follow facility safety program, identify and report unsafe equipment, environment or procedures to supervisor. Read and follow instructions on Safety Data Sheets (SDS) when handling or using chemicals. Assume accountability for data contained in the employee handbook. Observe all facility policies and procedures. Complete employee training courses as assigned by due date. Perform any other related duties as directed by supervisor.
- Must have current physical examination showing freedom from certain types of communicable diseases.
- Must be a graduate of an accredited COTA program.
- Must hold a valid Texas license to practice as an Occupational Therapy Assistant.
