Job summary

The Legacy Senior Communities is seeking an Activities (Lifestyles) Assistant - Memory Support for Plano, TX. The role supports the development and implementation of programs to engage residents in activities promoting mental, physical, and social well-being.

As a PRN position, responsibilities include coordinating with the Lifestyle Coordinator, conducting events, and ensuring resident safety and documentation. Minimum high school diploma and 6 months’ experience with seniors are required.

Qualifications

  • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
  • High School Diploma or equivalent.
  • At least 6 months of experience working with seniors and/or cognitively-impaired persons.

Responsibilities

  • Plan appropriate activities based on residents' interests and needs with the Lifestyle Coordinator.
  • Evaluate programming quality and participation via surveys and direct interaction with residents and families.
  • Coordinate communication with the Lifestyle Coordinator and staff while maintaining confidentiality.
  • Carry out large, small, and one-to-one events for friendly visitation, cognitive or sensory stimulation, and arrange transportation.
  • Recognize signs requiring nursing intervention and report changes to the appropriate channels.
  • Share pertinent information with staff and volunteers on a need-to-know basis.

About the Role

The LifestylesAssistant Assists with the development and implementation of programs that provide opportunities for residents to participate in events that maintain optimal levels of mental, physical, and social well-being.

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

  • Personally and through interview of staff members, evaluates the interest and abilities of residents and works with Lifestyle Coordinator to plan appropriate activities in terms of interest, needs and available personnel.
  • Evaluates programming quality and participation on an ongoing basis through surveys and direct interaction with residents and families. Encourages participation in resident council.
  • Communicates with the Lifestyle Coordinator on a regular basis to inform, exchange ideas, coordinate services and discuss format for individual and group programming. Also shares insight, information and reports responses and/or progress regarding residents on a need-to-know basis, maintaining confidentiality.
  • Carries out large, small, and one-to-one events for the purpose of providing any and all of the following: a friendly visitor; sensory stimulation; reality orientation; cognitive stimulation. Informs residents and staff to changes in scheduled programming in a timely manner. Arranges transportation for events.
  • Recognizes signs and symptoms which signify a change in condition that require nursing intervention and reports these changes to the appropriate channels. Shares resident information with staff on a need-to-know basis. Remains alert for conditions that threaten the safety and well being of staff and residents. Reports hazardous conditions to appropriate channels.
  • Shares pertinent information with staff and volunteers on need-to-know basis.
