Job summary

Gallatin Center for Rehabilitation and Healing in Tennessee seeks a compassionate Certified Occupational Therapy Assistant (COTA) to join our skilled nursing team. You will assist the OT with treatment plans, provide therapy services, and document progress to support resident independence.

The ideal candidate will hold a Tennessee COTA license, have completed an accredited OTA program, and excel in communication and collaboration with an interdisciplinary team.

Qualifications

• Current Tennessee COTA license is required.
• Graduate of an accredited OTA program.
• Experience in a skilled nursing or rehabilitation setting preferred.
• Strong communication and interpersonal skills.
• Compassionate, patient‑focused approach to care.
• Ability to work independently and as part of a team.

Responsibilities

• Assist the Occupational Therapist in implementing treatment plans for residents.
• Provide therapy services to help patients regain independence in daily living activities.
• Document treatment sessions accurately and timely per facility standards.
• Monitor and report patient progress or concerns to the Occupational Therapist.
• Maintain a clean, safe, and organized therapy environment.
• Collaborate with an interdisciplinary team to ensure optimal patient outcomes.
• Uphold all federal, state, and facility policies and procedures.

• Monitor and report patient progress or concerns to the Occupational Therapist

Certified Occupational Therapy Assistant (COTA) Qualifications:
