Nexus at Elmwood in Elmwood Park, IL, seeks a Certified Occupational Therapist Assistant (COTA) to work under the supervision of an Occupational Therapist. You will implement individualized treatment plans to help residents perform daily living activities, improve safety, and promote independence.

This role emphasizes education for residents, families, and staff, collaboration with a care team, and adherence to PCC or similar documentation practices; prior SNF experience preferred.

Qualifications

Certified Occupational Therapist Assistant (COTA)

Position Overview:

Works under the supervision of a licensed Occupational Therapist to provide rehabilitative services that help residents improve or maintain their ability to perform daily living activities. The COTA implements individualized treatment plans to promote independence, safety, and overall quality of life.

Key Responsibilities:

• Plan and administer treatment programs for residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.

• Provide skilled occupational therapy interventions in accordance with physician orders and the established plan of care, under the supervision of the supervising Occupational Therapist.

• Educate residents, families, and staff regarding therapy goals, techniques, and outcomes.

• Demonstrate competent technical skills consistent with current COTA standards of practice.

Preferred Qualifications:

• Prior experience in a skilled nursing facility (SNF) or long-term care setting preferred.

• Strong knowledge of current treatment practices and therapy interventions.
