Job summary

Caring Hand Home Health Solutions, Inc. is seeking an Occupational Therapist to help patients regain independence and function in their homes, delivering care that supports day-to-day self-care activities.

This per diem role offers a $130 per visit rate, with flexible scheduling and opportunities to convert to full-time based on performance. Mileage reimbursement, paid sick days, bereavement leave, and ongoing skills training are included.

Qualifications

• Graduate of an accredited occupational therapy program.
• Registered with the National Registration Examination (NBCOT).
• Active California OT license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver's license with insured vehicle and ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
