Job summary

Ascend Hospice in Stroudsburg, PA is seeking a Certified Nursing Assistant for weekend PRN shifts. You will provide ADL support, assist with toileting, hygiene, grooming, feeding, and comfort measures to hospice patients.

Must have a State Certified CNA license; hospice or home health experience preferred; willing to travel and have a valid driver's license and reliable transportation to fulfill visits.

Qualifications

• State Certified CNA license is required.
• Hospice or home health experience preferred.
• 1 year experience optional but preferred.

Responsibilities

• Provides or assists in ADL activities including personal hygiene, grooming, mouth, skin care and nails.
• Assists with toileting, positioning and transfer activities for bed-bound residents/patients.
• Helps with prescribed exercises and ROM as taught by Hospice Nurse.
• Performs light housekeeping of the unit, changes bed linen, and handles patient laundry.
• Feeds and assists with meals and monitors intake.
• Obtains temperature, pulse and respirations under supervision.
• Reports changes to Hospice Nurse and documents in patient records.
• Assists or reminds patients to take medications as regulated.

Now Hiring -Certified Nursing Assistant Weekend PRN- ( Stroudsburg, PA)

ThisCertified Nursing Assistantwill be responsible for, but not limited to:

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.

• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.

• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.

• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.

• Feeds and assists with meals.

• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.

• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.

• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education:

High School Diploma or equivalent

Professional Experience:

• Must have a State Certified CNA license

• Experience in hospice or home health, preferred

• No experience necessary but 1 year, preferred

• Must be willing to travel

• Must have a valid driver's license and reliable transportation
