Job summary

Encore Communities is seeking a Certified Nursing Assistant to provide personal care to residents under RN supervision in a caring, holistic environment.

Responsibilities include assisting with admissions, transfers, and discharges, and reviewing care plans daily to ensure alignment with resident needs. CNA licensure, CPR, and background clearance are required, with prior long-term care or rehab experience preferred.

Qualifications

• CNA license certification prior to hiring.
• Must have CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience
• LTC/SNF/Rehab preferred.

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
• Review care plans daily to determine if changes in the guests/patient’s daily care routine have been made on the care plan.
