Encore Northwoods Lodge in Washington's Silverdale area is seeking a Certified Nursing Assistant to support admissions, transfers and daily personal care under an evidence-based, collaborative care model.

Qualifications

• CNA license certification prior to hiring.
• Must have a current CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred but not required.
• Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience
• LTC/SNF/Rehab preferred.

Responsibilities

• Assist patients during admissions, transfers and discharges per care plan.
• Review daily care plans to adjust routines as needed.

Responsibilities

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
• Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan.
