Job summary

Affinity Hospice seeks a compassionate hospice aide (CNA) to work under RN supervision, providing essential personal care and comfort to patients in their homes. You will observe, report, and document patient conditions while supporting family needs and care plans.

The role requires lifting up to 50 lbs, CPR certification, and a valid driver’s license for local travel. This is a full-time, home-based position with potential for advancement and ongoing training.

Qualifications

• High school graduate or equivalent.
• Current CPR certification.
• Licensed driver with insured vehicle per organization requirements.
• Able to lift up to 50 lbs. and move patients.
• Ability to travel locally and work in patients’ homes.

Responsibilities

• Perform personal care tasks such as baths, back rubs, oral hygiene, shampoos, and changing bed linens as assigned.
• Assist in dressing and undressing patients as assigned.
• Assist in feeding the patient as assigned.
• Assess and report on patient condition and changes to the case manager.
• Adhere to documentation and care procedures and professional conduct standards.
• Do not administer medications or perform nursing procedures.
• Other CNA duties as assigned by management.

The hospice aide is responsible for observing the patient, reporting these observations and documenting observations and care performed.Hours / Schedule: Full Time; (40) hours / week. Works in patients' home in various conditions. Ability to work flexible schedule, ability to travel locally and ability to participate in holiday and weekend rotation as appropriate.Minimum Qualifications:High school graduate or equivalent.Meet the training and certification requirements of the State.Possess and maintain current CPR certification.Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.Able to show empathy for the needs of the ill, injured, frail and the impaired.Prolonged standing and walking required, with the ability to lift up to 50 lbs. and move patients.Key Responsibilities:Perform personal care needs after given a patient's assignment and attending to his/her requests promptly.Meet the safety needs of patients and use equipment safely and properly (foot stools, side rails, O2 etc.)Perform personal care tasks including baths, back rubs, oral hygiene, shampoos, and changing bed linen as often as assigned.Assist in dressing and undressing patients as assigned.Assist in feeding the patient as assigned.Assess and report on patients’ condition and significant changes to the case manager.Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.The hospice aide will not function in any manner, viewed as the practice of nursing according to the Nurse Practice Act.
