Job summary

Affinity Hospice is seeking a hospice aide (CNA) to join our team. The role supports patients in their homes under RN supervision, performing personal care and documenting observations to promote comfort.

The position is PRN with a need for flexible scheduling, potential travel locally, and participation in weekend/holiday rotations. Requires high school diploma, CPR, and a valid driver’s license.

Qualifications

  • High school diploma or equivalent.
  • State requirements training and certification.
  • Current CPR certification.
  • Licensed driver with insured vehicle.
  • Ability to lift up to 50 lbs and stand/walk for prolonged periods.
  • Ability to show empathy for ill, injured, frail.

Responsibilities

  • Perform personal care tasks as assigned and attend to patient requests promptly.
  • Follow safety procedures with equipment (foot stools, side rails, O2).
  • Assist in baths, back rubs, oral hygiene, shampoos, and changing bed linens.
  • Assist in dressing/undressing and feeding the patient as assigned.
  • Assess and report patient condition and changes to the case manager.
  • Document observations and care performed.

Position Summary

The hospice aide (CNA) is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for observing the patient, reporting these observations and documenting observations and care performed.

Hours / Schedule

PRN. Works in patients' home in various conditions. Ability to work flexible schedule, ability to travel locally and ability to participate in holiday and weekend rotation as appropriate.

Minimum Qualifications

High school graduate or equivalent. Meet the training and certification requirements of the State. Possess and maintain current CPR certification. Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order. Able to show empathy for the needs of the ill, injured, frail and the impaired. Prolonged standing and walking required, with the ability to lift up to 50 lbs. and move patients.

Key Responsibilities

Perform personal care needs after given a patient's assignment and attending to his/her requests promptly. Meet the safety needs of patients and use equipment safely and properly (foot stools, side rails, O2 etc.) Perform personal care tasks including baths, back rubs, oral hygiene, shampoos, and changing bed linen as often as assigned. Assist in dressing and undressing patients as assigned. Assist in feeding the patient as assigned. Assess and report on patients’ condition and significant changes to the case manager. Adhere to the organization's documentation and care procedures and standards of personal and professional conduct. The hospice aide will not function in any manner, viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a licensed nurse, such as sterile techniques. Perform other CNA duties as assigned by management to support department goals and initiatives.
