The Care Team Hospice in Peoria, IL is seeking a compassionate Certified Nursing Assistant (CNA) to join our growing team. This role offers a schedule with no nights, no weekends, and no on-call, serving patients within about an hour of the branch.

You will provide hands-on personal care and comfort in patients' homes or care facilities, collaborating with an interdisciplinary team to maintain dignity and quality of life.

Formación

• Active Certified Nursing Assistant (CNA) certification.
• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.

Responsabilidades

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).

• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
