Job summary

The Care Team Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our team in Champaign & Danville. This role provides hands-on personal care to hospice patients in homes and care facilities, helping maintain dignity, comfort, and quality of life while supporting families through difficult moments.

No nights, no weekends, no on-call schedule; includes tuition reimbursement for LPN/RN advancement and mileage reimbursement.

Qualifications

• Active CNA certification is required.
• Experience in hospice, home health, long-term care, or skilled nursing is preferred.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).
