Job summary

The Care Team Hospice in Ohio is seeking a compassionate Certified Nursing Assistant to join our growing team, delivering hands-on private and hospice care in patients' homes and facilities.

You will work with an experienced interdisciplinary team to maintain dignity, comfort and quality of life, with no nights or weekends and a one-hour radius from the branch. This role emphasizes patient-centered care, family support and consistent documentation in Homecare Homebase.

Qualifications

• Active CNA certification required.
• Hospice, home health, or long-term care experience preferred.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).

The Care Team Hospice is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing team!!
