Carbondale Rehabilitation and Healthcare Center LLC is seeking a Certified Nursing Assistant for 2nd shift (2PM-10PM) to provide compassionate overnight resident care in a skilled nursing facility.

The role emphasizes timely response to call lights, ADL assistance, mobility support, and accurate documentation, with a focus on safety, nourishment, and emotional support during the night.

Qualifications

• Active Pennsylvania CNA certification required.
• Strong communication and teamwork skills.
• New graduates welcome; experience in skilled nursing or long-term care preferred but not required.

Responsibilities

• Deliver excellent, compassionate care to residents during the night.
• Answer call lights promptly and respond to needs with patience and respect.
• Assist with activities of daily living (bathing, dressing, grooming, toileting).
• Support mobility, transfers, and ambulation safely.
• Turn and reposition residents to promote comfort and prevent skin issues.
• Assist with meals and ensure proper nutrition and hydration.
• Observe residents closely and report changes in condition to nursing staff.
• Accurately measure and document vital signs, intake/output, and other information.
• Maintain a clean, safe, and respectful environment.
• Provide emotional support and companionship during quieter hours.

What You’ll Do

As a CNA on 2nd Shift, you will:
