Job summary

Phoenix Home Care & Hospice seeks a Hospice Aide in Springfield, MO and surrounding areas. Provide personal care, bathing, and daily living support, coordinating with the nurse case manager to honor the care plan and deliver compassionate end-of-life care.

Qualifications

Responsibilities

• Document care provided according to agency policy.
• Report deviations from the Plan of Care to the Case Manager.
• Use Durable Medical Equipment safely.
• Maintains records of services performed and of apparent condition of patient.

We provide compassionate and dignified care while maintaining continuity of care. Hospice aides play a crucial role by providing support with personal hygiene and bathing. They collaborate closely with the nurse case manager to guarantee that all the patients' needs are being met in accordance with the established care plan.

Job Duties

• Perform personal care (bathing, dressing, toileting, feeding, etc.) according to the Hospice Aide Plan of Care.
• Document care provided according to agency policy.
• Report any deviations from the Plan of Care to the Case Manager (or designee if not available.)
• Use Durable Medical Equipment safely.
• Maintains records of services performed and of apparent condition of patient.
