Job summary

First Choice Home Health & Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to provide one-on-one personal care to patients in their homes on a consistent Monday–Friday morning schedule. Part-time, 15–20 hours/week, with a $2,000 sign-on bonus and paid travel time plus mileage reimbursement.

Qualifications

• Current active CNA certification.

• At least 18 years old.
• Valid driver's license.

Responsibilities

• Assist patients with bathing, grooming, dressing, and personal hygiene.
• Help with mobility and other activities of daily living.
• Support patient comfort, dignity, safety, and independence.
• Observe and report changes in patient condition.
• Communicate with nurses, patients, families, and the care team.
• Follow individualized plans of care.
• Complete accurate and timely documentation.
