Job summary

Diablo Valley Post Acute is seeking compassionate, dependable CNAs to join our 200-bed skilled nursing and post-acute care team in Pleasant Hill, CA. This full-time role offers $21.00-$23.00 per hour DOE, with a strong emphasis on resident-centered care.

Qualifications

• Current, active California CNA certification required.
• Previous skilled nursing or healthcare experience preferred, but not required.
• Newly certified CNAs are encouraged to apply.

Responsibilities

• Provide compassionate, high-quality care while promoting residents' dignity, comfort, safety, and independence.
• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, mobility, and meals.
• Follow individualized care plans and perform assigned nursing-related duties under the direction of licensed nursing staff.
• Observe residents for changes in condition and promptly report concerns to the nursing team.
• Accurately document care provided and maintain required records.
• Help maintain a clean, safe, and comfortable environment for residents.
• Assist with transfers, repositioning, ambulation, and other mobility needs.
• Communicate effectively with residents, families, and members of the interdisciplinary care team.
• Work collaboratively with nurses, therapists, and other healthcare professionals to support resident care goals.
