Job summary

Evergreen Healthcare Group's Polson Health and Rehabilitation Center in Polson, MT is seeking a Certified Nursing Assistant (CNA) to perform a range of duties caring for residents in the center, aligned with the plan of care and long-term care standards.

The role includes assisting with ADLs, feeding residents, transporting and turning residents, taking vital signs and documenting as directed; requires current CNA certification in Montana and a HS diploma; strong teamwork.

Qualifications

• CNA certification in Montana is required.
• High school diploma or equivalent required.
• Ability to work well in a team setting.

Responsibilities

• Answer signal lights.
• Assist with bathing, dressing, toiletry, and oral hygiene (ADLs).
• Serve and feed residents requiring help.
• Transport residents using wheelchairs or assist to walk.
• Turn and re-position residents as needed.
• Take and record vital signs, intake/output, and weight; document as directed.

Job Summary

Polson Health and Rehabilitation Centeris looking forCertified Nursing Assistant (CNA)to perform a combination of duties in caring for residents in the center, consistent with the plan of care and established long-term care standards.

Essential Functions:

• Answers signal lights
• Provides assistance with bathing, dressing, toiletry, and oral hygiene activities of daily living (ADLs).
• Serves and collects food trays and feeds residents requiring help.
• Transports residents, using wheelchair or wheeled cart, or assists residents to walk.
• Turns and re-positions residents, alone or with assistance
• Takes and records vital signs, food and fluid intake and output, and weight, as directed.
• Completes documentation

Qualifications:

• Required:Current certification as a nursing assistant in the state of Employment
• Required:High school graduate or equivalent
• Ability to work well in a team setting
