Job summary

Brighten Life Home Health is seeking a Physical Therapist to join our home health team. You will provide compassionate, one-on-one care to patients in their homes, helping them recover and maintain independence.

Flexible part-time and PRN scheduling support work-life balance while delivering high-quality patient care. You will collaborate with an interdisciplinary team and document care plans, with pay based on experience.

Qualifications

• Must be at least 18 years of age.
• Recent clinical experience or CNA training.
• Strong communication and interpersonal skills.
• Ability to meet employment screening requirements.

Responsibilities

• Assist patients with activities of daily living.
• Provide personal care, hygiene, and oral care services.
• Assist with dressing, grooming, and bathing needs.
• Take and record vital signs.
• Help maintain a clean, safe, and comfortable home environment.
• Document care provided according to the patient's plan of care.
