Responsibilities
- Performs personal care needs as assigned, attending to patient requests promptly, and reports to the Case Manager if unable to perform a task.
- Provides positive communication with patients and families, maintaining confidentiality and ethical standards.
- Ensures safety using equipment properly, including foot stools, side rails, and oxygen.
- Provides personal care such as baths, back rubs, oral hygiene, shampoos, and changing bed linens.
- Assists in dressing and undressing patients.
- Plans and prepares nutritious meals, including shopping, as assigned.
- Assists with feeding patients as needed.
- Records vital signs when ordered, demonstrating competency.
- Monitors patient’s skin to prevent tissue breakdown.
- Reports changes in patient’s condition to the Case Manager.
- Assists with ambulation, exercise, and positioning.
- Reminds and assists with self-administered medications, as permitted.
- Provides assistance with bedpans, urinals, light laundry, and simple procedures as ordered.
- Provides respite for patients, families, and caregivers when on-site.
- Keeps the patient’s living area clean and orderly.
- Follows documentation, care procedures, and standards of conduct.
- Participates in quality improvement activities.
- Respects patient and family needs and environment.
Qualifications
- Meets state training requirements.
- Completes at least 75 hours of classroom and supervised practical training, including 16 hours of practical training prior to certification.
- Minimum of six months experience providing personal care.
- At least 18 years old.
- Ability to read, follow instructions, and document care.
- Understanding of hospice philosophy and comfort providing care to terminally ill patients.
- Satisfactory references.
- Ability to work independently.
- Comfortable with issues of death and dying.
- Current CPR certification.
- Demonstrates tact, patience, and good hygiene.
- Successfully completes a probationary period to demonstrate competence.
- Strong communication and organizational skills.
