Responsibilities

• Performs personal care needs as assigned, attending to patient requests promptly, and reports to the Case Manager if unable to perform a task.
• Provides positive communication with patients and families, maintaining confidentiality and ethical standards.
• Ensures safety using equipment properly, including foot stools, side rails, and oxygen.
• Provides personal care such as baths, back rubs, oral hygiene, shampoos, and changing bed linens.
• Assists in dressing and undressing patients.
• Plans and prepares nutritious meals, including shopping, as assigned.
• Assists with feeding patients as needed.
• Records vital signs when ordered, demonstrating competency.
• Monitors patient’s skin to prevent tissue breakdown.
• Reports changes in patient’s condition to the Case Manager.
• Assists with ambulation, exercise, and positioning.
• Reminds and assists with self-administered medications, as permitted.
• Provides assistance with bedpans, urinals, light laundry, and simple procedures as ordered.
• Provides respite for patients, families, and caregivers when on-site.
• Keeps the patient’s living area clean and orderly.
• Follows documentation, care procedures, and standards of conduct.
• Participates in quality improvement activities.
• Respects patient and family needs and environment.

Qualifications

• Meets state training requirements.
• Completes at least 75 hours of classroom and supervised practical training, including 16 hours of practical training prior to certification.
• Minimum of six months experience providing personal care.
• At least 18 years old.
• Ability to read, follow instructions, and document care.
• Understanding of hospice philosophy and comfort providing care to terminally ill patients.
• Satisfactory references.
• Ability to work independently.
• Comfortable with issues of death and dying.
• Current CPR certification.
• Demonstrates tact, patience, and good hygiene.
• Successfully completes a probationary period to demonstrate competence.
• Strong communication and organizational skills.
