Job summary

Evergreen Healthcare Group is seeking a Certified Nursing Assistant (CNA) in La Grande, Oregon to provide compassionate care to residents in accordance with the plan of care and long-term care standards.

You will assist with ADLs, monitor vital signs, document care, and work as part of a dedicated nursing team in a setting that offers Private Health Care Plan, DailyPay, life insurance, and growth opportunities. This role requires current CNA certification in Oregon and a high school diploma.

Job Summary

La Grande Post Acute Rehab is looking for Certified Nursing Assistant (CNA) to perform a combination of duties in caring for residents in the center, consistent with the plan of care and established long-term care standards.

Essential Functions:

• Answers signal lights
• Provides assistance with bathing, dressing, toiletry, and oral hygiene activities of daily living (ADLs).
• Serves and collects food trays and feeds residents requiring help.
• Transports residents, using wheelchair or wheeled cart, or assists residents to walk.
• Turns and re-positions residents, alone or with assistance
• Takes and records vital signs, food and fluid intake and output, and weight, as directed.
• Completes documentation

Qualifications:

• Required:Current certification as a nursing assistant in the state of Employment
• Required:High school graduate or equivalent
• Ability to work well in a team setting
