Carlton Shores Health and Rehab Center in Daytona Beach, FL is seeking a Certified Nursing Assistant to join our compassionate team. Provide direct resident care under supervision and support daily living activities while promoting a respectful environment.

The position requires Florida CNA certification, CPR preferred, and at least one year of skilled nursing experience. Excellent English communication, professionalism, and teamwork are essential.

Responsibilities

• Provide personal care (grooming, bathing, dressing, oral care) of residents daily and as needed.
• Change bed linen and maintain grooming supplies in resident rooms daily.
• Ambulate and transfer residents using appropriate devices and body mechanics.
• Record resident vitals, weights, intake/output, and food consumption as assigned.
• Routinely turn and position residents as necessary.
• Observe residents and report changes to Charge Nurse.
• Assist in data collection for MDS and care plan; provide input to nurses.
• Provide adequate hydration to residents and assist with meals as needed.
• Keep accurate records of food intake and output.
• Monitor ventilation, temperature, light, and noise levels.
• Take TPR, BP, and weights as directed.
• Answer call lights promptly and assist with other resident needs.
• Assist in orientation and training of other staff as requested.

Summary for Certified Nursing Assistant (CNA) Perform direct resident care duties under the supervision of licensed nursing personnel. Assist with promoting a compassionate physical and psychosocial environment for the residents.

Responsibilities

• Provide personal care (i.e., grooming, bathing, dressing, oral care, etc.) of residents daily and as needed.
• Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis.
• Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics.
• Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned.
• Routinely turn and position residents, as necessary.
• Observe residents carefully and report changes in condition to Charge Nurse.
• Participate in collecting data needed for the accurate completion of the MDS and the plan of care. Provide input to the nurses for development of the plan of care.
• Provide adequate hydration to residents at regular intervals each day according to specific resident needs.
• Prepare residents for each meal and transport to dining room (unless a bed patient) making sure that food tray is accessible, self-help devices are available (if needed), and assistance is provided to those residents who cannot feed themselves.
• Keep an accurate record of food intake and output, as directed.
• Be alert for proper ventilation, temperature, light and noise control.
• Take TPR (temperature, pulse, & rate), blood pressure, weighing of residents, etc., as directed.
• Promptly answer call lights and assist with other resident needs.
• Assist in the orientation and training of other staff, as requested.
