Job summary

The Ponce Therapy Care Center in St. Augustine, FL, is hiring CNAs to provide compassionate care and support to residents. A $2,000 sign-on bonus for full-time or $1,000 for part-time is offered, along with a welcoming team and modern facility.

Responsibilities include direct resident care, documenting vitals and intake/output, assisting with meals and mobility, and supporting care planning under supervision. Florida CNA certification and good English communication are required.

Qualifications

Responsibilities

• Provide personal care for residents daily under licensed nursing supervision.
• Change bed linens and maintain supplies in the resident room.

• Record vital signs, weights, intake/output, and food consumption.
• Turn and position residents and monitor comfort.
• Observe residents and report changes to the Charge Nurse.
• Assist in data collection for MDS and care plans.
• Ensure adequate hydration and assist meals as needed.
• Record food intake and output accurately.
• Monitor environment for proper ventilation, temperature, lighting.
• Take TPR, BP, and other vital measures as directed.
• Respond promptly to call lights and resident needs.
• Assist in orientation and training of other staff as requested.

Summary

Perform direct resident care duties under the supervision of licensed nursing personnel. Assist with promoting a compassionate physical and psychosocial environment for the residents.

Requirements / Qualifications

• Current Nursing Assistant certification from state of Florida.
• Current/active CPR certification, preferred.
• At least one (1) year Skilled Nursing experience preferred.
• Effective verbal and written English communication skills.
• Highest level of professionalism with the ability to maintain confidentiality.
• Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
• Customer service oriented with the ability to work well under pressure.

Responsibilities

• Provide personal care (i.e., grooming, bathing, dressing, oral care, etc.) of residents daily and as needed.
• Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis.
• Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics.
• Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned.
• Routinely turn and position residents, as necessary.
• Observe residents carefully and report changes in condition to Charge Nurse.
• Participate in collecting data needed for the accurate completion of the MDS and the plan of care. Provide input to the nurses for development of the plan of care.
• Provide adequate hydration to residents at regular intervals each day according to specific resident needs.
• Prepare residents for each meal and transport to dining room (unless a bed patient) making sure that food tray is accessible, self-help devices are available (if needed), and assistance is provided to those residents who cannot feed themselves.
• Keep an accurate record of food intake and output, as directed.
• Be alert for proper ventilation, temperature, light and noise control.
• Take TPR (temperature, pulse, & rate), blood pressure, weighing of residents, etc., as directed.
• Promptly answer call lights and assist with other resident needs.
• Assist in the orientation and training of other staff, as requested.
