Job summary

Elara Caring is seeking a Certified Nurse Aide to join our team, delivering personal care and daily living assistance in patients' homes across Indiana. You will work under a Registered Nurse, perform vital signs, assist with mobility, and ensure safety and comfort in the home environment.

Responsibilities include following individual care plans, documenting care activities, and reporting changes to the care team. Valid CNA certification and reliable transportation are required.

Qualifications

• Current State certificate as a Certified Nursing Aide.
• Ability to frequently lift up to 50 pounds, including transferring patients and moving equipment.
• Must possess a valid drivers license or have reliable transportation to travel between patient homes.

• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patients individualized care plan.
• Monitor and report changes in a patients condition to the care team.
• Maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation, protect patient privacy, and participate in required training.

What is Required?

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
