Job summary

Elara Caring is seeking a Certified Nurse Aide to provide compassionate in‑home care to patients in Elkhart, IN. You will work under a Registered Nurse to carry out individualized care plans, assist with daily activities, monitor vital signs, and document changes.

Qualifications

• Current state CNA certification.

Responsibilities

• Provide hands‑on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and daily activities.
• Take and record vital signs and monitor patient condition.
• Help patients safely walk and transfer using mobility equipment.
• Follow individualized care plans and maintain documentation.
• Report changes to the care team and ensure a clean, safe home environment.
• Remind patients to take medications as directed and respond to emergencies.

Job Description

As a Certified Nurse Aide, you’ll contribute to our success in the following ways:

• Provide hands‑on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient’s individualized care plan.
• Monitor and report changes in a patient’s condition to the care team.
• Help maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation, protect patient privacy, and participate in required training.

What is Required?

• Current State certificate as a Certified Nursing Aide.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
