Job summary

Continuum Hospice is seeking a Full Time Hospice Aide (CNA) in Barnstable, MA. You will work with the nursing team to provide compassionate end-of-life care, focusing on comfort, dignity, and patient and family support.

The role requires a CNA certificate, CPR if mandated, a high school diploma, and at least 1 year of CNA experience in acute or long-term care. A valid driver’s license and transportation are required. Competitive benefits and a sign-on bonus are offered.

Qualifications

Responsibilities

• Provide personalized care according to the plan of care and attend to patient and family requests.
• Assist with personal care tasks such as bathing, hygiene, changing bedding, and dressing to maintain comfort and dignity.
• Deliver exceptional customer service and contribute to patient satisfaction and comfort.
• Support families by sharing knowledge and guiding them in caring for loved ones.
• Maintain accurate documentation in patient records and comply with agency standards.

Are you a caring and dedicated Certified Nursing Assistant looking to make a meaningful impact? Continuum Hospiceis seeking a Full Time Hospice Aide (CNA) to join our compassionate team, providing hands-on care that brings comfort and dignity to patients at the end of life. Working closely with our nursing team, you’ll ensure each patient’s physical, emotional, and personal needs are met with respect, empathy, and professionalism.

Schedule: Monday - Friday, 8 am - 4:30 pm

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well-being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.
• Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction. Your dedication to providing outstanding care will be valued and celebrated as part of our team.
• Provide Support for Families: Impact patients' families by sharing your knowledge and expertise, guiding and supporting them in caring for their terminally ill loved ones. Your presence offers confidence and reassurance during this challenging time.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state-mandated
• Current driver's license, vehicle insurance, and access to dependable transportation.

$23.00 - $26.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
