Job summary

Care 360 Hospice in Toledo, OH is looking for a compassionate Certified Nursing Assistant to join our patient-centered team. You will provide personal care, help with bathing, grooming, dressing, toileting, and feeding according to the hospice plan of care, and travel to patients in residences and facilities.

The role includes offering emotional support to patients and families, assisting with transfers and mobility, using DME, documenting care, and participating in monthly trainings.

Qualifications

• Must hold a state CNA license.
• CPR/BLS certification.
• Experience in AL/LTC/SNF or home health required.
• Willing to travel.
• Valid driver's license with reliable transportation.

Responsibilities

• Provide personal care (bathing, grooming, dressing, toileting, feeding) per the hospice plan of care.
• Travel to patients' residences and facilities to deliver hospice services.
• Offer emotional support and companionship to patients and families.
• Provide bedside vigil support during end-of-life care.
• Assist with ambulation, transfers, positioning, and ROM as documented.
• Safely use and assist with DME (walkers, wheelchairs, hospital beds, lifts).
• Observe and report changes to the case manager or supervising nurse.
• Document care accurately per policies.
• Participate in team meetings and staff conferences.
• Support family members and caregivers within CNA scope.
• Attend monthly educational meetings and ongoing training.
• All other duties as assigned.

Hospice Certified Nursing Assistant

Salary: $19-22/hr BOE
Schedule: Full-time | Monday - Friday
Shift Details: 3 PM - 11 PM + OC rotation every 8-10 weeks
Location: Toledo, Ohio, and surrounding areas

Requirements of the CNA role:

Hospice Experience Preferred

• Licensed Nursing Assistant in the state of practice (CNA)
• CPR/BLS Certified
• Prior experience working in an AL/LTC/SNF facility or Home Health REQUIRED
• Enjoy working with Senior Citizens, Positive Attitude, Professional, Responsible & Dependable
• Must be willing to travel
• Valid driver's license and Auto insurance Coverage with reliable transportation

About the Certified Nursing Assistant role:

• Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care.
• Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner.
• Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life.
• Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments.
• Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care.
• Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts.
• Observe and report changes in the patient’s physical condition, emotional status, or home environment to the case manager or supervising nurse.
• Accurately document care provided in accordance with Care 360 Hospice policies and procedures.
• Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences.
• Provide support and education to family members and caregivers within the scope of the CNA role.
• Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance.
• All other duties as assigned
