Job summary

The Care Team Home Health & Hospice is seeking a Certified Nursing Assistant (CNA) for the 2nd shift in Akron, OH. This role offers no weekends or nights, with tuition reimbursement for LPN/RN advancement and a strong benefits package.

You will deliver compassionate patient care within a one-hour radius of the branch, supporting families and maintaining quality of life in a hospice setting. Qualifications include active CNA certification and prior hospice/home health experience preferred.

• Reliable transportation and clean driving record may be required.

Responsibilities

• Provide personal care, bathing, grooming, and comfort measures per patient care plan.
• Observe and report changes in condition to supervising nurse.
• Support patients and families with compassion and respect; promote comfort and quality of life.
• Assist with mobility and activities of daily living; maintain safe environment.
• Document all patient care promptly in the EMR (Homecare Homebase).

Job Location: Akron Cuyahoga Falls Hospice - Akron , OH, Salary Range: Undisclosed, Certified Nursing Assistant (CNA) 2nd Shift - No Weekends. Tuition Reimbursement for LPN/RN Path.

Schedule, Hours & Territory

2nd Shift - Start at 12-2p Ending 8-10p. Flexible schedule!!

• No Weekends
• No On-Call

Serving patients within approximately a one-hour of Branch

Qualifications: Key Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).

Qualifications

• Active Certified Nursing Assistant (CNA) certification
• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.

Field Role Requirements

• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to pass a criminal background check and Motor Vehicle Record (MVR) screening.
