Job summary

Care 360 Hospice is seeking a compassionate Hospice Certified Nursing Assistant to travel to patients’ homes and facilities in Toledo, OH and surrounding areas. The role emphasizes end-of-life comfort, dignity, and quality of life, with a schedule of 3 PM - 11 PM, full-time.

You will provide personal care, monitor conditions, document care per policy, and collaborate with the care team and family members. CNA license and CPR/BLS are required; hospice experience preferred.

Qualifications

• Licensed Nursing Assistant (CNA) in the state of practice.
• CPR/BLS certification.
• Prior experience in AL/LTC/SNF or Home Health required.
• Hospice experience preferred.
• Must be willing to travel.

Responsibilities

• Provide compassionate, patient-centered personal care (bathing, grooming, dressing, toileting, feeding).
• Travel to patients’ residences and facilities to deliver hospice services with dignity.
• Provide bedside vigil support during end-of-life care as needed.
• Assist with ambulation, transfers, positioning, and ROM exercises.
• Safely use DME (walkers, wheelchairs, beds, lifts) as required.
• Observe and report changes to the case manager or supervising nurse.
• Document care accurately per Care 360 Hospice policies.
• Participate in team meetings and educate family members within CNA scope.
• Attend monthly on-site training and maintain competency.
• All other duties as assigned.
