Job summary

PACE of Montgomery County, part of Trinity Health PACE, is seeking a Certified Nurse Assistant to provide hands-on care and support for participants in a home-care setting. You will assist with ADLs, mobility, and supervision, while promoting independence and safety within an interdisciplinary team.

The role requires Maryland CNA certification, a valid driver’s license, and 1 year of geriatric care experience or PACE training.

Qualifications

• High school diploma or equivalent required.
• Active Nurse Assistant certification in Maryland.
• Valid driver’s license and vehicle for home care assignments.
• Experience in long-term care or geriatric programs preferred.
• At least 1 year of experience with frail/elderly or PACE training at hire.
• Proficiency with Microsoft Office and email/calendar systems.
• Physical ability to kneel, crouch, reach, lift up to 20 pounds.

Responsibilities

• Provide personal care to maintain independence in ADLs (toileting, showers, eating, grooming).
• Assist with activity, exercise, and walking.
• Provide behavioral supervision and reminders (medication, diet, reassurance).
• Assist with conversations, recreation, meals, snacks, and housekeeping.

Employment Type: Full time Shift: Day Shift Description: Full Time, Monday-Friday Pay Range: $18.30-$21.96 Sign On Bonus - $1,500 Position Purpose: The Certified Nurse Assistant is responsible for providing various care services to meet the personal needs, support, and health status of participants.

What You Will Do

• Provide personal care assistance to maintain participants' independence in activities of daily living, including toileting, showers, eating, and grooming.
• Assist participants in maintaining physical activity, exercise, and walking.
• Offer behavioral supervision or support, including reality orientation, medication reminders, dietary restrictions, and emotional reassurance.
• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

Minimum Qualifications

• High school diploma or equivalent required.
• Active certification as a Nurse Assistant in Maryland.
• Valid driver’s license and use of personal vehicle required for home care assignments.
• Experience in a long-term care facility or community-based geriatric program preferred.
• Minimum of one (1) year of documented experience working with a frail or elderly population, or appropriate training from PACE organization upon hiring.
• Strong conflict management and customer service skills, adaptable to changing work priorities.
• Proficiency with Microsoft Office suite and electronic mail/calendar systems.
• Ability to engage in physical tasks such as kneeling, crouching, reaching, lifting, sitting, walking, bending, and lifting up to 20 pounds.
