Job Title: Certified Nursing Assistant

Location: Brooklyn, New York, United States - 11212

Pay Range: $21.00 - $24.00/hr

Shift: Days, Monday-Friday (7 AM to 3 PM/ 3 PM to 11 PM / 11 PM to 7 AM)

Duration: 06 Months (Contract) with possible extension

Duties:

• Personal Hygiene & Grooming: Assists residents with daily hygiene, including bathing, oral care, hair grooming, shaving, nail care, and dressing, ensuring cleanliness and dignity are maintained at all times.
• Incontinence & Skin Care: Provides incontinence care, skin care, and supports bowel and bladder monitoring programs; records outputs and reports irregularities to nursing staff.
• Mobility & Transfers: Aids in repositioning, ambulation, transfers using mechanical lifts or mobility aids, and supports range-of-motion exercises and positioning to prevent pressure injuries and contractures.
• Resident Observation & Reporting: Monitors residents for physical, behavioral, or cognitive changes, responds to call bells and alarms promptly, and communicates all significant observations to nursing staff.
• Vital Signs & Documentation: Takes and records vital signs, weights, and meal intake/output using EMR systems; completes shift documentation and reports care delivered.
• Feeding & Hydration: Prepares residents for meals, assists with feeding, encourages fluid intake, monitors nutritional intake, and promptly informs nurses of inadequate intake/output.
• Resident Support & Engagement: Provides compassionate care to terminally ill, isolated, or cognitively impaired residents, encourages daily participation, and assists with activities and appointments.
• Housekeeping & Facility Support: Maintains clean resident rooms and service areas, identifies equipment needing repair, conserves supplies, and assists with admissions, discharges, and emergency duties as assigned.
