Job summary

Ascend Hospice & Palliative Care MO seeks a Certified Nursing Assistant to join our compassionate team in Missouri. The role supports daily resident care, ADLs, hygiene, mobility, meals, and safe medication reminders.

You will work Monday–Friday, 8:30 AM–5:30 PM, with travel to care sites as needed; a valid driver's license and reliable transportation are required. We offer competitive pay and strong benefits as part of Ascend’s patient-centered mission.

Qualifications

• State Certified CNA license is required.
• Hospice or home health experience preferred.
• No experience necessary, but 1 year preferred.
• Willingness to travel to care sites.
• Valid driver's license and reliable transportation required.

Responsibilities

• Assist residents with ADL, personal hygiene, grooming and nails.
• Assist with toileting, transfers, and positioning.
• Support prescribed exercises/ROM as taught.
• Light housekeeping of the unit and patient laundry.
• Assist with meals and feeding.
• Take temperature, pulse, and respirations under supervision.
• Report changes to Hospice Nurse and document communications.
• Remind and assist with medication per regulations.

Certified Nursing Assistant (CNA) - Lee's Summit, MO

Hours: Monday-Friday 8:30AM-5:30PM

This Certified Nursing Assistant will be responsible for, but not limited to:

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education:

High School Diploma or equivalent

Professional Experience:

• Must have a State Certified CNA license
• Experience in hospice or home health, preferred
• No experience necessary but 1 year, preferred
• Must be willing to travel
• Must have a valid driver's license and reliable transportation
