Job summary

Ascend Hospice & Palliative Care MO in Lee's Summit, MO is seeking a Certified Nursing Assistant (CNA) for a Monday–Friday, 8:30 AM–5:30 PM role to support patients in home health or facility settings.

You will assist with ADLs, toileting, transfers, ROM, meals, and vitals, while documenting and reporting changes to the Hospice Nurse. A state CNA license is required, with willingness to travel and a valid driver's license.

Qualifications

• State Certified CNA license required.
• High school diploma or equivalent.
• Hospice or home health experience preferred.
• Willingness to travel; valid driver's license and reliable transportation.

Responsibilities

• Assist with ADLs and personal care.
• Assist with toileting, transfers, and turning.
• Support ROM exercises as taught.
• Light housekeeping of unit and patient laundry.
• Feed and assist with meals.
• Obtain vitals under nurse supervision.
• Report changes to nurse and document appropriately.

Certified Nursing Assistant (CNA) - Lee's Summit, MO

Hours: Monday-Friday 8:30AM-5:30PM

This Certified Nursing Assistant will be responsible for, but not limited to:

1. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
2. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
3. Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
4. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
5. Feeds and assists with meals.
6. Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
7. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
8. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education

High School Diploma or equivalent

Professional Experience

• Must have a State Certified CNA license
• Experience in hospice or home health, preferred
• No experience necessary but 1 year, preferred
• Must be willing to travel
• Must have a valid driver's license and reliable transportation
