Job summary

Ascend Hospice & Palliative Care MO in Lee's Summit, MO is seeking a Certified Nursing Assistant (CNA) to join our hospice team. This on-site role supports residents with ADL, personal care, and daily activities, Monday through Friday 8:30 AM to 5:30 PM.

The ideal candidate will hold an active state CNA license, have hospice or home health experience preferred, and be willing to travel with reliable transportation. High school diploma or equivalent required.

Responsibilities

• Provides ADL assistance including personal hygiene, grooming, and nail care.
• Assists with toileting, transfers, and positioning; turns and positions bed-bound residents.
• Assists with prescribed exercises as taught by Hospice Nurse.
• Light housekeeping of resident unit; changes bed linen and patient laundry as needed.
• Feeds and assists with meals.
• Records vital signs under supervision of Hospice Nurse.
• Reports changes in patient condition to Hospice Nurse and facility staff when applicable.
• Assists or reminds patient to take medications in compliance with regulations.

Certified Nursing Assistant (CNA) - Lee's Summit, MO

Hours: Monday-Friday 8:30AM-5:30PM

This Certified Nursing Assistant will be responsible for, but not limited to:

1. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
2. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
3. Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
4. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
5. Feeds and assists with meals.
6. Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
7. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
8. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education

High School Diploma or equivalent

Professional Experience

• Must have a State Certified CNA license
• Experience in hospice or home health, preferred
• No experience necessary but 1 year, preferred
• Must be willing to travel
• Must have a valid driver's license and reliable transportation
