Job summary

Care 360 Hospice is seeking a compassionate Certified Nursing Assistant to provide patient-centered care in home and facility settings around Dayton, Ohio. You will travel to patient locations to deliver hospice services, support families, and ensure comfort at end of life.

The role requires a valid CNA license and BLS, with travel willingness and reliable transportation. Hospice experience is preferred and growth opportunities are available.

Qualifications

• Licensed Nursing Assistant in the state of practice (CNA).
• BLS certification completed.
• Prior experience in AL/LTC/SNF or Home Health is REQUIRED.
• Willingness to travel and drive to patient locations.
• Valid driver’s license and auto insurance with reliable transportation.
• Hospice experience preferred.

Responsibilities

• Provide compassionate, patient-centered personal care per plan of care.
• Travel to patients’ residences and facilities to deliver hospice services.
• Offer emotional support and companionship to patients and families.
• Provide bedside vigil support during end-of-life care as needed.
• Assist with ambulation, transfers, positioning, and ROM exercises.
• Use and assist with DME like walkers and hospital beds safely.
• Observe changes and report to case manager or supervising nurse.
• Document care per agency policies and participate in team meetings.
• Educate family members and caregivers within CNA scope.
• Attend on-site monthly trainings and upkeep competency.

Hospice Certified Nursing Assistant

Salary: $19-22/hr BOE

Schedule: Monday-Friday 7 AM- 3:30 PM

Location: Kettering, Ohio (surrounding area)

Requirements of the CNA role:

• Licensed Nursing Assistant in the state of practice (CNA)
• BLS Certified
• Prior experience working in an AL/LTC/SNF facility or Home Health REQUIRED
• Enjoy working with Senior Citizens, Positive Attitude, Professional, Responsible & Dependable
• Must be willing to TRAVEL
• Valid driver's license and Auto insurance Coverage with reliable transportation
• Hospice experience preferred

About the Certified Nursing Assistant ro le:

• Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care.
• Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner. Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life.
• Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments.
• Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care.
• Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts.
• Observe and report changes in the patient’s physical condition, emotional status, or home environment to the case manager or supervising nurse.
• Accurately document care provided in accordance with Care 360 Hospice policies and procedures.
• Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences.
• Provide support and education to family members and caregivers within the scope of the CNA role.
• Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance.
• All other duties as assigned
