Job summary

Care 360 Hospice in Toledo, OH is seeking a dedicated Certified Nursing Assistant to provide compassionate personal care in patient homes and facility settings. This full-time role includes 8:00 AM–4:30 PM shifts with occasional on-call weekend rotation every 8–10 weeks.

You will travel to patients' residences, assist with activities of daily living, and offer emotional support to patients and families while maintaining hospice care standards.

Qualifications

• Licensed Nursing Assistant in the state of practice (CNA).
• CPR/BLS certified.
• Prior experience in AL/LTC/SNF or Home Health required.
• Enjoy working with seniors with a positive attitude and dependability.
• Must be willing to travel.
• Valid driver's license and auto insurance with reliable transportation.
• Hospice experience preferred.

Responsibilities

• Provide compassionate, patient-centered personal care (bathing, grooming, dressing, toileting, feeding) per the hospice aide plan.
• Travel to patients' residences (homes and facilities) to deliver hospice services.
• Offer emotional support and companionship to patients and families.
• Provide bedside vigil support during end-of-life care as needed.
• Assist with ambulation, transfers, positioning, and ROM as documented.
• Safely use DME (walkers, wheelchairs, beds, lifts).
• Observe and report changes to the case manager or nurse.
• Document care provided per Care 360 Hospice policies.
• Collaborate with the inter-disciplinary team and participate in staff meetings.
• Educate and support family members within CNA scope.
• Attend monthly on-site education and training to maintain competency.

Hospice Certified Nursing Assistant

Salary: $19-22/hr BOE

Schedule: Full-time | Monday - Friday

Shift Details: 8 AM-4:30 PM + On Call Weekend Rotation every 8-10 weeks

Location: Toledo, Ohio, and surrounding areas

Requirements of the CNA role:

• Licensed Nursing Assistant in the state of practice (CNA)

• Enjoy working with Senior Citizens, Positive Attitude, Professional, Responsible & Dependable


• Must be willing to travel

About the Certified Nursing Assistant role:

• Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care.


• Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner.


• Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life.


• Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments.


• Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care.


• Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts.


• Observe and report changes in the patient’s physical condition, emotional status, or home environment to the case manager or supervising nurse.


• Accurately document care provided in accordance with Care 360 Hospice policies and procedures.


• Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences.


• Provide support and education to family members and caregivers within the scope of the CNA role.


• Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance.
