Job summary

Care 360 Hospice is seeking a dedicated Hospice Certified Nursing Assistant for a full-time role in Southern Dayton, Ohio, including Springboro and surrounding areas. The position runs Monday–Friday, 7:00 am–3:30 pm, with travel to patients' residences as part of hospice care.

Requirements include an active CNA license in Ohio, BLS certification, and prior AL/LTC/SNF or Home Health experience. Travel and reliable transportation are required; hospice experience is preferred.

Qualifications

• Must be a Licensed Nursing Assistant (CNA) in the state of practice.
• BLS certification required.
• Prior experience in AL/LTC/SNF facility or Home Health required.
• Willingness to travel and reliable transportation.
• Hospice experience preferred.

Responsibilities

• Provide compassionate, patient-centered personal care per the hospice aide plan of care.
• Travel to patients’ residences to deliver hospice services.
• Offer emotional support and companionship to patients and families.
• Provide bedside vigil support during end-of-life care as needed.
• Assist with ambulation, transfers, and range of motion exercises.
• Safely use durable medical equipment (DME) such as walkers and wheelchairs.
• Observe and report changes to the case manager or supervising nurse.
• Document care provided in accordance with Care360 Hospice policies.
• Participate in on-site monthly educational meetings and ongoing training.
• Support family members and caregivers within the CNA scope.
• All other duties as assigned.

Hospice Certified Nursing Assistant

Salary: $19-22/hr BOE

Schedule: Full time | M-F| 7 am-3:30 pm

Location: Southern Dayton, Ohio, and surrounding areas

Including: Springboro/ Lebanon/ Middletown

Requirements of the Certified Nursing Assistant role:

• Licensed Nursing Assistant in the state of practice (CNA)
• BLS Certified
• Prior experience working in an AL/LTC/SNF facility or Home Health REQUIRED
• Enjoy working with Senior Citizens, Positive Attitude, Professional, Responsible & Dependable
• Must be willing to TRAVEL
• Valid driver's license and Auto insurance Coverage with reliable transportation
• Hospice Experience Preferred

About the CNA role:

Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care.

Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner.

Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life.

Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments.

Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care.

Safely use and assist patients with durable medical equipment (DME) such as walkers, wheelchairs, hospital beds, and lifts.

Observe and report changes in the patient’s physical condition, emotional status, or home environment to the case manager or supervising nurse.

Accurately document care provided in accordance with Care 360 Hospice policies and procedures.

Foster a cooperative team environment by understanding the roles of interdisciplinary team members and actively participating in team meetings and staff conferences.

Provide support and education to family members and caregivers within the scope of the CNA role.

Participate in required on-site monthly educational meetings and ongoing training to maintain competency and compliance.
