Job summary

Heritage-Woods-of-Centralia is seeking a dedicated CNA to provide individualized care according to each resident’s plan. You will assist with ADLs, promote independence, and maintain dignity while reporting health changes to the clinical team.

Engage with residents and families in a compassionate, impactful manner and participate in ongoing education, staff meetings, and confidentiality protocols. Comprehensive benefits and growth opportunities are available.

Qualifications

• High School graduate or GED equivalent required.
• Current certification from an accredited CNA program.
• Exceptional communication and interpersonal skills.
• Demonstrate ability to serve with love, compassion, and dignity.

Responsibilities

• Deliver individualized care per each resident’s care plan.
• Assist with Activities of Daily Living (ADLs) while promoting independence and dignity.
• Report health status changes to the clinical team.
• Engage with residents and family to create a positive experience.
• Participate in ongoing education and team meetings.
• Maintain confidentiality of resident information as required.
