Qualifications

• Follow all care plan directives and provide direct patient care.

Responsibilities

• Assist residents with daily living activities (feeding, bathing, dressing, grooming, transfers).
• Transfer residents to dining rooms and activities as scheduled.
• Document vital signs and care provided with accuracy and timeliness.
• Ensure safety and comfort for residents during all care interactions.
• Communicate changes in condition to nursing team promptly.
