Job summary

VitalCaring Group is seeking a Hospice Aide/CNA to provide comfort, dignity, and compassionate care to patients in their homes. You will work closely with RNs, social workers, and the care team to support end-of-life comfort and family needs.

The role requires state-certified Hospice Aide/CNA training, current CPR, and the ability to follow care plans with precise documentation. Travel between patient homes and a patient-first mindset are essential.

Qualifications

• Completion of state-required Hospice Aide/CNA training.
• Current CPR certification.
• Ability to follow care plans, document clearly, and communicate effectively.
• Reliable transportation and ability to travel between patient homes.
• True patient-first mindset with respect for dignity and quality of life.

Responsibilities

• Assist patients with daily living activities including bathing, dressing, grooming, toileting, and feeding.
• Support comfort through repositioning, mobility assistance, and skin integrity care.
• Provide light housekeeping to maintain a clean, safe, and comfortable environment.
• Offer respite support to caregivers and families.
• Observe and report changes in patient condition to the RN/Case Manager.
• Assist with vital signs and basic care tasks as directed.
• Document care accurately and timely.

What You’ll Do

What You Bring
