Job summary

Meadow Lakes is seeking a Certified Home Health Aide (CHHA) to join our team in East Windsor, NJ. This full-time role covers night shifts 11-7am and weekend hours 7-3pm, assisting patients with daily living activities under supervision of licensed staff.

The CHHA will monitor patient conditions, provide companionship, help with transfers, hygiene, meals, and mobility support, while documenting care and communicating with the care team. NJ CHHA license and high school diploma are required.

Qualifications

• High school diploma or equivalent required.
• Valid NJ CHHA License required.

Responsibilities

• Observe patient health conditions and maintain daily records for family or supervisor.
• Guide patients in prescribed exercises per care plan.
• Assist with transferring, bathing, grooming, and daily activities.
• Maintain records of patient care and report observations to supervisor.
• Entertain or read to patients to support mental health and well-being.
• Serve meals per prescribed diets and assist with prep and cleanup.
