The role involves personal care, mobility assistance, health monitoring, meal support, light housekeeping, companionship, and medication administration within scope.

Qualifications

• Valid New Jersey CHHA certification is required.
• Weekend availability is required.
• Prior home care or related healthcare experience is preferred.
• Reliable transportation to client homes in Hamilton, NJ.
• Strong interpersonal and communication skills with a patient-centered approach.
• Ability to follow individualized care plans and work independently.
• CPR/First Aid certification is a plus.

• Provide personal care (bathing, grooming, hygiene) to clients.
• Assist with mobility, transfers, and ambulation.
• Monitor and document health status; report changes to supervising nurse.
• Prepare meals and assist with feeding per care plans.

Responsibilities

• Provide personal care assistance including bathing, grooming, dressing, and hygiene support
• Assist clients with mobility, transfers, and ambulation as needed
• Monitor and document client health status, reporting any changes to the supervising nurse
• Prepare meals and assist with feeding in accordance with dietary needs and care plans
• Perform light housekeeping duties to maintain a safe and clean home environment
• Offer companionship and emotional support to promote client well-being
• Administer medications or treatments as directed within scope of practice

Requirements

• Valid New Jersey Certified Home Health Aide (CHHA) certification required
• Prior experience in home care, assisted living, or a related healthcare setting preferred
• Availability to work weekends (Saturday and/or Sunday)
• Reliable transportation and ability to travel to client homes in the Hamilton, NJ area
• Strong interpersonal and communication skills with a patient-centered approach
• Ability to follow individualized care plans and work independently
• CPR/First Aid certification is a plus
