Job summary

Vitality Hospice & Palliative Care is seeking a compassionate Certified Home Health Aide (CHHA) to join our clinician-owned and operated hospice. This full-time, field-based role supports patients in their homes with dignity and quality of life, under the supervision of an RN Case Manager.

You’ll perform mobility support, personal care, companionship, and light housekeeping while documenting care and following care plans. Immediate hire available.

Qualifications

• Active CHHA certification required.
• Valid driver’s license, reliable vehicle, and current auto insurance.
• Compassionate, reliable, and self-directed.
• Strong communication and time-management skills.
• Ability to work independently and follow care plans.
• Commitment to serving patients with chronic, serious, or terminal illnesses.

Responsibilities

• Working under the supervision of a Registered Nurse Case Manager, you will provide essential hands-on care and support, including:

Certified Home Health Aide (CHHA) – Full-Time | Field-Based

Vitality Hospice & Palliative Care is seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our growing team. This is an opportunity to work for a clinician-owned and operated hospice that truly values both its patients and its employees.

Position Details

• Schedule: Full-Time, Monday–Friday, typically 7:00 AM–3:00 PM
• Occasional evening/weekend overtime available
• Field-based position with travel required (mileage reimbursed)
• Active CHHA certification required

What You’ll Do
