Vitality Hospice is seeking a compassionate Certified Home Health Aide (CHHA) to join our clinician-owned team. This full-time, field-based role supports patients in their homes across the community, with travel required and mileage reimbursement.

You will work under the supervision of a Registered Nurse Case Manager, providing mobility assistance, personal care, companionship, and light housekeeping while documenting care and following care plans. Active CHHA certification is required.

Vitality Hospice & Palliative Care is seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our growing team. This is an opportunity to work for a clinician-owned and operated hospice that truly values both its patients and its employees.

Position Details

• Schedule: Full-Time, Monday-Friday, typically 7:00 AM–3:00 PM
• Occasional evening/weekend overtime available
• Field-based position with travel required (mileage reimbursed)
• Active CHHA certification required

What You'll Do

Working under the supervision of a Registered Nurse Case Manager, you will provide essential hands‑on care and support, including:

• Assisting with mobility, transfers, and ambulation
• Providing personal care (bathing, grooming, dressing, feeding)
• Offering companionship and emotional support

Compensation

$18.00–$23.00 per hour , based on experience and qualifications
