Vitality Hospice & Palliative Care is seeking a compassionate Certified Home Health Aide (CHHA) to join our clinician-owned team. This full-time, field-based role involves working under a RN Case Manager to provide hands-on care in patients' homes.

The position offers mileage reimbursement, a Monday–Friday schedule (7:00 AM–3:00 PM on average), and comprehensive benefits. Active CHHA certification, valid driver’s license, and reliable vehicle are required.

Certified Home Health Aide (CHHA) – Full-Time | Field-Based

Vitality Hospice & Palliative Care is seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our growing team. This is an opportunity to work for a clinician-owned and operated hospice that truly values both its patients and its employees.

Position Details

• Schedule: Full-Time, Monday–Friday, typically 7:00 AM–3:00 PM
• Occasional evening/weekend overtime available
• Field-based position with travel required (mileage reimbursed)
• Active CHHA certification required

What You’ll Do

Working under the supervision of a Registered Nurse Case Manager, you will provide essential hands‑on care and support, including:

• Assisting with mobility, transfers, and ambulation
• Providing personal care (bathing, grooming, dressing, feeding)
• Offering companionship and emotional support
• Performing light housekeeping tasks
• Documenting care accurately and in a timely manner
• Supporting a culture of compassion, quality, and respect

You will be an integral member of our interdisciplinary team focused on clinical excellence and patient-centered care.

Compensation

$18.00–$23.00 per hour, based on experience and qualifications
