Vitality Hospice & Palliative Care is seeking a compassionate Certified Nursing Assistant (CNA) for a full-time, field-based role serving patients in their homes. You will provide hands-on care, support comfort and quality of life, and work under a RN Case Manager in a team-focused environment.

Typical schedule is 7:00 AM – 3:00 PM, with occasional evenings/weekends and mileage reimbursement. Immediate hiring with opportunities for growth and benefits packages.

Qualifications

• Active CNA certification required.
• Valid driver's license, reliable vehicle, and current auto insurance.
• Ability to work independently in the field.
• Experience with hospice, palliative care, or chronic illness preferred (not required).

Responsibilities

• Assist patients with mobility, transfers, and ambulation.
• Provide personal care including bathing, grooming, dressing, and feeding.
• Offer companionship and emotional support to patients and families.
• Perform light housekeeping tasks related to patient care.
• Accurately document care provided in a timely manner.

Certified Nursing Assistant (CNA) – Full-Time | Field-Based

Vitality Hospice & Palliative Care

Vitality Hospice & Palliative Care is a clinician-owned and operated hospice seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing interdisciplinary team.

In this field-based role, you will provide hands‑on care and emotional support to patients in their homes, helping preserve comfort, dignity, and quality of life during their end‑of‑life journey. Schedule & Position Details

• Full-Time, Monday–Friday
• Typical hours: 7:00 AM – 3:00 PM
• Occasional evening/weekend overtime available
• Field-based role with local travel
• Mileage reimbursement provided

Key Responsibilities

Under the direction of a Registered Nurse Case Manager, the CNA will:
